Please list any medical conditions we should be made aware of (i.e. usage of an Epi-pen*, allergies, ADHD, ADD, physical disabilities, emotional problems, learning disabilities or anything special we should know about your child. If your child has an allergy, please list signs, symptoms, and treatment.

This form is to confirm your spot. Once received and processed you will receive a confirmation email, payment amount, and all registration forms.

We ask that once your form is submitted, please send a $25 registration to confirm your spot for this summer. This amount will be deducted from your final cost.

Our accounting office will determine your final amount.
